Pre-Participation Physical Examinations (PPE)

What Happens During Sports Physical?
Your child’s sports physical examination, mandated by the specific sport’s organizers or governing body, may encompass:
- Blood pressure, oxygen saturation, heart rate, and rhythm assessment
- Information about dietary supplements, medications, or prescriptions for your child
- Evaluation of flexibility, joint health, posture, and overall strength
- Comprehensive physical examination covering the ears, nose, throat, heart, lungs, and abdomen
- Measurement of height and weight
- Review of the athlete and family’s medical history
- Vision assessment
What should I bring to a sports physical?
Before your visit, please ensure you have any required forms provided by your school or organization. Both you and your parent/guardian should complete the necessary fields in advance.
In case you don’t have a form, you can download our standardized physical form (PDF) from the provided link. This form is recognized and recommended by experts in pediatric and sports medicine.
Additionally, gather the following information before your visit:
- Primary care provider’s name and contact details.
- Comprehensive medical history, including any allergies and up-to-date immunizations.
- If you have a history of a heart ailment or cardiac event, please obtain a statement from your primary care provider or treating physician. This statement should confirm that the condition has been evaluated and does not hinder your participation.

Keith Lemmon, a pediatrician at Madigan Army Medical Center, Joint Base Lewis-McChord, conducted a global health engagement at Peleliu Elementary School in Peleliu, Palau. During this event, he imparted valuable lessons on resilience and mental health to the children. Exercise Palau is an initiative sponsored by U.S. Army Pacific and hosted by the nation of Palau. It plays a vital role in fortifying the relationships between the U.S. and Palau, while contributing to a free and open Indo-Pacific.
