Don’t worry about multiple repetative appointment like physical therapy. Make sure to include the major stuff like hospitalizations, physical exams, surgeries, that have happened over the 3 years.
You would need to pass an FAA exam (at least class 3) prior to being able to be eligible for Basic Med. Please see the FAA website regarding Basic Med here : https://www.faa.gov/licenses_certificates/airmen_certification/basic_med/. Also all instructions for basic Med with links are on our website
For the AME exams (Class 1/2/3), it is good to have a parent but not absolutely necessary. For HIMS <18, yes, a parent should be present.
Not for the Initial HIMS Consultation. You don’t need the MedXpress form until later after the first consultation. You should only bring any medical records related to your diagnosis and treatment to your HIMS visit if possible.
We use the Snellen eye chart for near and distant vision and the Ishihara plates for color vision
Shihara plates.
As long as you are making progress toward what the FAA has requested of you, you can request an extension. The FAA usually grants extensions for a month at a time. Feel free to reach us via the Contact Us Form or at 253-313-0935 and we would be happy to assist you with getting an extension!
